Coverage is available for $80,000, $135,000 or $165,000 of protection.

No evidence of good health is required if you enroll during the open enrollment
period or within 31 days of your date of eligibility.

The"Accelerated Death Benefit Rider" allows for an early payment of 50% up to
$50,000 if you are terminally ill with a life expectancy of 12 months or less.
Your death benefit will be reduced by the amount you elect under this
provision.

Payment of premium can be waived if you are totally disabled for 6 months, you
are less then 60 years old when the disability begins, and you continue to be
totally disabled.

The amount of insurance reduces 35% at age 65, plus an additional 25% at age 70
and 20% at age 75.

Coverage will end on your termination of employment. You may convert your
insurance to a AXA Equitable Life Insurance Company individual life

Child Coverage

Dependent Term Life coverage has one premium rate that covers all eligible
children

Coverage is available for $4,000 for all your children from 14 days of age.

If your dependent children are confined for medical care or treatment at home
or elsewhere, coverage will begin when confinement ends.

Coverage begins at 14 days and continues to age 19, if unmarried. If the child
is unmarried, dependent on you and a full-time student, coverage continues to
age 26.

Coverage will end on your termination of employment. Insurance may be converted
to a AXA Equitable Life Insurance Company individual life insurance
policy. *Important Notice: Tax laws relating to acceleration of life benefits
are complex. Your are advised to consult with a qualified tax advisor about
circumstances under which you could receive acceleration of life insurance
benefits that are excludable from income under federal laws.

Seat Belt/Airbag Benefit - The plan pays an additional
benefit of 10% of your coverage amount.
Air Bag Benefit - The plan pays an additional benefit of
10% of your coverage amount. The total maximum benefit payable is $100,000.00
Exclusions - A loss is not covered if it results from
suicide or attempted suicide; intentionally self-inflicted injuries or an
attempt at same; sickness; medical or surgical treatment of sickness; act of
war; certain full-time military duty; commission of, or attempt to commit a
felony; intoxication or drug use; injury rising out of, or in the course of any
work for wage or profit, certain travel or flight in a vehicle used for aerial
navigation.
MEMBER Coverage

Your coverage amount is equal to your Optional Term life coverage amount.

Benefits are paid in addition to Optimal Term Life benefits.
